West Yellowstone School

West Yellowstone School will be hosting free sports physicals provided by Billings Clinic that will cover your student for the 25-26 school year, on May 9^th & 23^rd. You must fill out the Physical Evaluation Consent form and the parent part of the MHSA Physical form prior to your appointment (Attached below). If your student uses glasses or contacts be sure the have them at the time of the physical. If your student is 12 or under a parent/guardian must be present.
